- A tuition invoice or statement with your name and date.
- A signed letter or email from your instructor with your name and date.

In the form’s Website Username field, include the username associated with your EclipseCAT.com account.

3 What Happens Next
Once our team receives your completed form and approves your proof of enrollment, you will receive an email containing information about your license, software download instructions, and the benefits available to Advantage Software users.
